Application Form Checklist

Have you attached the following documents to your completed application form?

• $150.00 administration fee  
• Copy of most recent school report
• Copy of NCEA qualifications and/or record of learning
• Copy of birth certificate for New Zealand citizens
• Copy of NZ passport/other passport showing permanent residency or citizenship document for students born out of New Zealand
• Evidence of recent participation in co-curricular activities listed on front page
• One small recent photograph - passport size. (Please insert in space on front page of application).

It is essential that copies NOT originals are attached to safeguard against possible loss of valuable documents. These will not be returned.
