Epsom House Open Day is Sunday 25th March 2012, 1.00-3.00pm

Have you attached the following documents to your completed application form?

$50.00 administration fee
Copy of most recent school report
Copy of NCEA qualifications and/or record of learning
Copy of birth certificate for New Zealand citizens
Copy of NZ passport/other passport showing permanent residency or citizenship document for students born out of New Zealand
Evidence of recent participation in co-curricular activities listed on front page
Two small recent photographs - passport size. (Please insert one in space on front page of application).

It is ESSSENTIAL that COPIES not ORIGINALS are attached to safeguard against possible loss of valuable documents. (These will not be returned)
